SAMPLE WELLBEING INSIGHTS AUDIT
You will be asked a sequence of questions regarding your experience at work which should take around 10 minutes to complete. This is part of the wellbeing journey at [YOUR ORGANISATION] and is your opportunity to give us your thoughts on where we should prioritise our efforts. Some of the questions may appear to be similar but they based on whether you experience the statement and how often it occurs.
Please be assured of confidentiality and anonymity, the survey is run by an external impartial agency. If you would prefer to complete the survey in private, please scan the QR code with a separate device.  [YOUR ORGANISATION] will not receive any individual or identifiable data.
